While paper forms will be accepted, we ask that you complete this form and if not using online payment, submit anyway so that the process of making your badge is easier and more streamlined. Online Payment Clause

Online payments are convenent but come with a small cost. If paying online, the price includes a 2.9% card processing fee of the total cost, plus $0.30. The prices are already factored into the form.
